2010 Kindergarten Enrollment
Beginning March 1, 2010, schools throughout Simi Valley will be pre-registering students for kindergarten classes that will begin in August. If you have a child who will be entering kindergarten, it is very important that your child be pre-registered. To be eligible for kindergarten in August, your child must be five (5) years of age on or before December 2, 2010. Registration packets may be picked up in the school office between 9:00 a.m. and 3:30 p.m. beginning March 1st.
The following items must be submitted by the first day of school or your child may not attend class:

OFFICIAL DOCUMENT VERIFYING DATE OF BIRTH - certified copy of birth
certificate (hospital birth certificates will not be accepted), or
passport.
• PROOF OF RESIDENCY: Document showing your name and address that will verify
residency in Sycamore’s attendance area – i.e., utility bill, lease agreement, escrow statement
• CURRENT IMMUNIZATION RECORD:
Polio Vaccine: 4 doses -OR- 3 doses with the last dose given after the 4th birthday
DPT: 5 doses -OR- 4 doses with the last dose given after the 4th birthday
MMR: 2 doses - both given after the 1st birthday
Hepatitis B 3 doses
Varicella 1 dose – given after the 1st birthday OR a doctor’s note
(Chickenpox) verifying your child has already had chickenpox

This form is available in the Kindergarten registration packet or by clicking on the link above. The form must be filled out and signed by your doctor and returned to Sycamore before school starts in August. Physicals given prior to March 1, 2010 will not be accepted. (**If your child's fifth birthday is after school begins in August and your insurance company will not pay for a physical exam until your child reaches five (5) years of age, you will be required to sign a Waiver of Health Examination for School Entry form until such time the physical examination is completed.)
PROPER IMMUNIZATIONS AND A PHYSICAL ARE REQUIRED FOR SCHOOL ATTENDANCE AND MUST BE ON FILE IN THE SCHOOL OFFICE. CHILDREN WILL BE EXCLUDED FROM SCHOOL UNTIL SUCH TIME AS THEY ARE PROVIDED.
